The game uses a top-down, mouse-driven perspective. Each hospital begins with limited space and resources, and the player must build rooms such as GP's Offices, Pharmacies, and treatment facilities before hiring staff and admitting patients.

Patients arrive with different illnesses, and diagnosis leads them through the appropriate treatment path. The player can place equipment, set staff policies, adjust salaries, research cures, and expand into new rooms or plots. Scenarios introduce increasingly difficult hospitals, special conditions, and financial targets.
