The game uses an isometric, mouse-driven perspective. Each scenario begins with a hospital plot and objectives such as reaching a financial target, curing enough patients, or maintaining a required reputation.

Players place diagnostic and treatment rooms, hire doctors, nurses, handymen, receptionists, and other staff, then manage patient flow through the hospital. Research unlocks improved treatments and equipment, while emergencies, epidemics, weather, and staff needs create additional pressure. Completing a scenario unlocks later hospitals with larger layouts and more demanding conditions.
