The game uses a side-view perspective and presents a linear sequence of scrolling stages. Spider-Man and Venom must defeat successive groups of enemies, avoid hazards, and survive boss encounters tied to the Maximum Carnage storyline.

Spider-Man is more agile and uses web-based attacks, while Venom is stronger and relies on heavier physical strikes. Players can call selected allied heroes for limited attacks, collect health and other items, and continue through a password-based progression system.
